Recently I benefited from Alex sharing his student survey questions in this community. To reciprocate, I want to share the survey questions that I developed off of his to assess our tablet program. The link to see the survey at surveymonkey.com is:

One of the nice features of surveymonkey is that you can send your survey questions to other administrators with surveymonkey accounts and they can then add to or modify them for their own schools. I would be willing to do this if you direct-message me.
The results from my students were interesting. They basically validate what we are doing, expressing positive satisfaction and usage patterns, but there are common concerns about damage, startup & logon time, and distraction.
I'm preparing a similar survey for faculty and if you've done this would appreciate your thoughts on good questions to ask.
